- Your email address, only if the form asks for it (it is optional)
- The department or doctor you choose
- The message you write, for example your health problem or a preferred day

We use these details to call you or WhatsApp you back, to arrange or confirm your appointment and to answer your question.
Your consent
By sending the form, you agree that we may use these details to contact you about your request. You can withdraw consent at any time by calling or emailing us, and we will stop using your details for this purpose, except for any record the law requires us to keep. If you fill the form for a family member, please make sure they agree.
Where it is kept and for how long
Form submissions are stored in a database on our hospital's web server, and only those members of our staff who need them to contact you or arrange your care should see them. We keep your information only as long as needed for your care and legal record-keeping, and then delete it.
Your rights
Under the DPDP Act you can ask us for the following. If you are not satisfied with our reply, you may then approach the Data Protection Board of India.
- Access: a summary of the personal information we hold about you and how we use it
- Correction: fix, complete or update information that is wrong

If the patient is under 18, a parent or legal guardian should fill the form and give consent for the child.
