A fistula in ano (भगंदर) is a small tunnel between the inside of the anus and the skin nearby. It usually starts with an anal abscess, which is a painful collection of pus near the anus. When the abscess bursts or is drained, a track can remain that keeps leaking pus or blood-stained fluid.
A fistula rarely heals on its own. The tunnel has an opening inside the anus, so infection keeps entering it. Antibiotics may calm the swelling for a while, but they cannot close the tunnel.
Our aim is to clear or seal the whole track while protecting the sphincter muscles that give you control over stool and gas. Depending on the type of fistula, we do this by fistulotomy, fistulectomy, a seton, or in selected cases by laser closure of the track (FiLaC).

We begin with a gentle, private examination. We check blood sugar, because diabetes slows healing. For complex, deep or recurrent fistulas, our surgeon may advise an MRI fistulogram, a special MRI scan that maps the tunnel.
In a fistulotomy, a simple, low track is opened and left to heal from the inside. In a fistulectomy, the tunnel is removed. High or complex fistulas may be treated in stages with a soft thread called a seton. Laser closure (FiLaC) seals the cleaned track from inside with a laser fibre and spares the muscle, but it does not heal every fistula, and a repeat or another procedure may be needed.
After fistulotomy or fistulectomy, the wound is usually left open so that it heals from the inside, which helps prevent the fistula from coming back. Take warm sitz baths two to three times a day and after every stool, and change the dressing as we teach you. Take the stool softener and pain medicines as advised, eat fibre and drink plenty of water. Most people return to light work in 1 to 2 weeks. An open wound often takes about 4 to 6 weeks to heal, and larger ones longer; wounds after laser closure are smaller. Please keep every follow-up visit with us.

A fistula can come back in some patients, especially when it is complex, high or long-standing, and a further procedure may be needed. When a track passes through the sphincter muscle, there is a risk of reduced control over gas or stool. Laser closure spares the muscle, but its healing rate is lower than that of fistulotomy, so some patients need a repeat or another procedure. We lower these risks by mapping the track carefully and following you up until the wound heals.

Eat soft, fibre-rich home food such as dal, roti, green vegetables, fruits and salad, with plenty of water, so that the stool stays soft and does not hurt the wound. We may advise isabgol. Avoid very spicy and fried food, maida items and alcohol for the first few weeks. If you have diabetes, keep your sugar under control, because high sugar slows wound healing and raises the chance of the fistula coming back.
