What is a CBD stone operation (CBD exploration)?

The common bile duct (CBD) is the main pipe that carries bile from the liver and gallbladder into the intestine. Stones can slip into it from the gallbladder, or sometimes form inside it. A stone in the CBD can block the flow of bile. This causes jaundice (yellow eyes and skin), pain, and sometimes a serious infection of the bile duct called cholangitis.

In CBD exploration, our surgeon opens the bile duct, removes the stones, flushes the duct clean and checks that it is clear. We usually do this in the same operation as gallbladder removal (cholecystectomy), because the gallbladder is usually the source of the stones. Symptoms of bile duct stones: jaundice, pain and fever

Before treatment we usually check liver function tests (LFT), blood counts and clotting tests, and do an ultrasound, which may show a widened duct or stones. We often advise an MRCP, a special MRI scan of the bile ducts done at an imaging centre, to confirm the stones and plan treatment.

Duct stones need treatment because they can cause repeated jaundice, cholangitis or pancreatitis (swelling of the pancreas). Cholangitis, with fever, chills, jaundice and pain, is an emergency. It first needs antibiotics, fluids and drainage of bile. Some patients are treated with ERCP, an endoscopic procedure to remove duct stones done at centres that offer it, followed by gallbladder removal. For others, CBD exploration with gallbladder removal in one operation is the better choice. How is a CBD stone operation done?

  1. We first do tests: LFT, blood counts, clotting tests, ultrasound and MRCP. Any infection or clotting problem is treated before surgery.
  2. Our anaesthesia specialist gives general anaesthesia.
  3. By open or laparoscopic surgery, depending on your case, Dr. Thakur frees the gallbladder and exposes the common bile duct.

T-tube and wound care: if a T-tube is placed, keep the bag below the level of the wound, do not pull the tube, and note the colour and amount of bile as we teach you. Keep the wound clean and dry. Jaundice and itching usually settle over some days to weeks.
