An ovarian cyst is a fluid-filled sac on or inside the ovary. Many cysts are harmless. Functional cysts form as part of the monthly cycle and often go away on their own within a few months. Other cysts, such as a dermoid cyst, an endometrioma (chocolate cyst), or a large or long-lasting cyst, may need surgery.
Laparoscopic ovarian cystectomy means removing the cyst by keyhole surgery while keeping the healthy part of the ovary. We take special care over this in young women, because the ovary makes hormones and eggs. When the ovary is badly damaged, or in some older women, we may advise removing the whole ovary (oophorectomy).
We send every removed cyst for histopathology (examination under a microscope) to confirm exactly what it was. This report guides your further care.


To assess a cyst, we usually advise an ultrasound of the lower abdomen, blood tests, a pregnancy test when relevant, and sometimes a blood marker such as CA-125. Small, simple cysts often need only a repeat ultrasound after a few weeks or months.
We may advise surgery when a cyst is large, keeps growing, does not go away, causes pain, or looks complex on ultrasound. Emergency surgery is needed for ovarian torsion, when the ovary twists and its blood supply is cut off, and sometimes for a burst cyst with bleeding. Torsion causes sudden, severe lower abdominal pain, often with vomiting. In torsion, quick surgery gives the best chance of saving the ovary. If tests suggest the cyst may not be simple, we plan the treatment carefully and may take the opinion of our visiting surgical oncologist.
Most women walk the same day and eat light food as we advise. Shoulder or abdominal discomfort from the gas used during surgery may last 1 to 2 days. Light household work can usually be resumed in about 1 week, and heavy work after 2 to 4 weeks, as Dr. Thakur advises. Periods may be a little irregular for one or two cycles. Please come for follow-up with your histopathology report. We may advise a repeat ultrasound later, especially for cysts that can come back.
Diet: start with liquids, then light home-cooked food such as dal, rice, khichdi, curd, green vegetables and fruit. Drink enough water to avoid constipation. Avoid heavy, oily food for a few days.
Activity: walk the same day. Light household work can usually be resumed in about 1 week, and heavy work after 2 to 4 weeks, as your surgeon advises. Please avoid lifting heavy weights until Dr. Thakur says it is safe.
Wound care: keep the small cuts clean and dry, and bathe when our team allows. Shoulder or abdominal discomfort from the gas may last 1 to 2 days. Periods may be a little irregular for one or two cycles.

Like any operation, ovarian cyst surgery has possible risks. These include bleeding, infection, injury to nearby organs such as the bowel or bladder (rare), and spillage of cyst contents. Some cysts, such as endometriomas, can come back. Removing a cyst can slightly reduce the ovary's egg reserve, and Dr. Thakur keeps this in mind in young women. Our anaesthesia specialist gives general anaesthesia, and Dr. Rajnish Kumar Thakur, our chief surgeon, makes 3 small cuts and puts in a camera. He carefully separates the cyst from the healthy part of the ovary and removes it, often in a small bag so that its contents do not spill. If the ovary has twisted, we untwist it and keep it if it looks able to recover. We send the removed tissue for histopathology and close the cuts.
The operation usually takes 45 to 90 minutes. Most women stay in our hospital for 1 to 2 days. The stay can be longer after an emergency operation for torsion or a burst cyst with bleeding, or if Dr. Thakur has to change to open surgery for your safety. Torsion means the ovary twists on itself, cutting off its own blood supply. It usually causes sudden, severe pain on one side of the lower abdomen, often with nausea and vomiting.
