Hypospadias is a condition that a boy is born with. The urine opening is not at the tip of the penis but lower down on the underside, anywhere from immediately below the tip to the scrotum. The penis may also bend downwards (chordee), and the foreskin is often incomplete underneath, so it looks like a hood on top.
Hypospadias repair is an operation to build a urine tube up to the tip, straighten the penis if needed, and give a more normal appearance. This helps the boy pass urine in a straight stream while standing and, later in life, supports normal sexual function. Milder types are usually repaired in one operation. More severe types may need a staged repair, done in two operations some months apart.


Hypospadias is usually noticed at birth. Our surgeon examines the baby to see where the opening is, how much the penis bends, and whether both testes are in the scrotum. If a testis cannot be felt, or the hypospadias is severe, we may advise further tests before surgery.
We commonly perform surgery between 6 and 18 months of age, when anaesthesia is safe, the tissues are suitable, and the child will not remember the operation. Older children can also be treated.
Important for parents: do not get your son circumcised before our surgeon has seen him. The foreskin is often used to build the new urine tube, and circumcision before repair can make the operation harder. Read more about circumcision and undescended testis surgery.
Give pain medicines as advised. The dressing and catheter must be kept clean; many children wear two diapers so that the catheter drains into the outer one. Swelling and bruising of the penis are normal for a few weeks. Keep your child away from riding toys and rough play for a few weeks. We remove the catheter at a follow-up visit, after which you should watch the urine stream. Please come back to us if urine leaks from a new hole, the stream is very thin, or there is fever or bleeding.
Hypospadias repair is delicate surgery. Possible problems include swelling, bleeding, infection, a small leak hole along the new tube (fistula), narrowing of the new opening or tube, the wound opening up, and some remaining bend. Some of these need another operation, usually after about 6 months. We reduce these risks with careful technique, fine stitches, proper catheter care and regular follow-up.
The cost of hypospadias repair depends on the type and severity, whether it is a single operation or planned in two stages, the anaesthesia, the stay (usually 1 to 3 days), whether your son goes home with a catheter, dressings, medicines, the room type and follow-up visits. We commonly perform surgery between 6 and 18 months of age. At this age anaesthesia is safe, the tissues heal well, and the child does not remember the operation or feel embarrassed later. Staged repairs are usually started in this period, with the second stage some months later. If your son is older, the repair can still be done, so please bring him to our surgeon without delay.
The foreskin is valuable tissue for the repair. Our surgeon often uses it to build the new urine tube or to cover the repair. If the child is circumcised first, this tissue is lost and the operation becomes more difficult. If you notice that the urine opening is not at the tip, or the foreskin looks like a hood, please get the child checked by our surgeon before any circumcision.
Many boys with milder hypospadias need only one operation. More severe types are often repaired in two planned stages, several months apart. Sometimes a complication, such as a small leak hole (fistula) or a narrowing, needs a further operation, usually after about 6 months when the tissues have settled. After a successful repair, most boys pass urine in a straight stream while standing, and the repair is designed to allow normal sexual function in adult life. Fertility is usually not affected by hypospadias alone. Regular follow-up, including around puberty, helps find any problem, such as a narrowing or a bend, early so that it can be corrected.

Under general anaesthesia, with a local block for pain, our surgeon first straightens any bend in the penis. Then a new urine tube is made, often from nearby skin or foreskin, so that the opening comes to the tip. The repair is covered with healthy tissue and the skin is closed with fine dissolving stitches. A thin catheter and a dressing are kept for some days while it heals.
The operation usually takes 1 to 2 hours, depending on the type. Most children stay 1 to 3 days. Some go home with the catheter, which is removed at a follow-up visit after several days. Swelling and bruising of the penis are normal and settle over a few weeks. We usually remove the catheter after several days at a follow-up visit. Most children are back to normal play within 1 to 2 weeks, but riding toys and rough play should wait a few weeks. Full healing can take about 6 weeks. No. Hypospadias is a difference in how the penis formed before birth, and it does not correct itself with age, creams or medicines. Very mild forms with a straight penis and a good stream may sometimes be left alone after discussion with our surgeon. Most boys benefit from repair, so that they can pass urine standing in a straight stream and have normal function as adults.
It depends on the type and severity, one or two stages, the anaesthesia, the hospital stay, the catheter and dressings, medicines, the room type and follow-up visits.
