URS or URSL (ureteroscopic lithotripsy) treats stones stuck in the ureter, the thin tube that carries urine from the kidney to the bladder. A very thin telescope called a ureteroscope is passed through the urinary passage and the bladder up to the stone. There is no cut on the body. A lithotripsy device breaks the stone into small pieces, using energy such as laser or pneumatic energy, depending on the case and available equipment. The pieces are removed, or left small enough to pass out in the urine.
After URSL, our surgeon often places a DJ stent. This is a thin, soft plastic tube with a curl at each end, shaped like the letter J, which lies inside the body from the kidney to the bladder. It keeps urine flowing while the swelling settles. It is temporary and must be removed on the date we advise.


We usually advise URSL when a ureter stone is too big to pass, when pain or vomiting continues despite medicines, when the kidney is swelling, or when the stone has not moved after a few weeks. Tests include ultrasound, X-ray KUB, urine tests, kidney function and often a CT scan.
We may place a DJ stent alone, urgently, when a stone blocks an infected kidney. Fever with chills and stone pain needs care the same day. We then treat the stone after the infection settles.
Please do not forget your stent. A stent left in for too long can get coated with stone, get blocked, cause infection and damage the kidney, and removing it can then need bigger operations. Save the removal date on your phone and keep your discharge papers safe. For large kidney stones, PCNL may be the better option.
Most people walk the same day and eat normally. Some burning, blood-tinged urine and a need to pass urine often are common for a few days. With a DJ stent you may feel discomfort in the side while passing urine, a frequent urge to urinate, or discomfort at the end of the urinary passage. Drinking water and the prescribed medicines usually help. Light work is often possible within a few days. Please come to us urgently for high fever, severe pain, heavy bleeding or if you cannot pass urine. Please have the stent removed on the date we advise.
Possible risks include urine infection or fever, blood in the urine for a few days, discomfort from the stent, a piece of stone moving up into the kidney that needs another procedure and, rarely, injury to the ureter or its later narrowing. Sometimes the ureter is too narrow to reach the stone safely; we then place a stent and treat the stone after 1 to 2 weeks. The biggest avoidable risk is a forgotten stent, so please keep the removal date we give you.

A DJ stent is meant to be temporary. If it stays in much longer than advised, salts from the urine can form a crust or stone on it. It can then get blocked, cause repeated infections, break, or silently damage the kidney. Removing a forgotten stent can need several bigger operations. Please get it removed on the date written on your papers, even if you feel completely well.

Common feelings are a frequent urge to pass urine, mild burning, a pulling pain in the side while urinating, and pink urine, especially after physical activity.
