What is PCNL surgery for large kidney stones?

PCNL (percutaneous nephrolithotomy) is a keyhole operation for large kidney stones. Percutaneous means through the skin. Our surgeon makes a small cut, usually about 1 cm, in the back over the kidney and passes a thin telescope called a nephroscope directly into the kidney.

Through the nephroscope, a lithotripsy device breaks the stone into small pieces, using energy such as laser or pneumatic energy, depending on the case and available equipment. The pieces are then washed or lifted out. At the end, our surgeon may leave a DJ stent (a thin tube inside the body, from the kidney to the bladder) and sometimes a small drainage tube from the back, called a nephrostomy, for a day or two, so that urine drains well while the kidney heals.

Signs of a large kidney stone

When is PCNL surgery in Gopalganj advised?

How is PCNL done? Step by step

  1. Tests and planning: we arrange an ultrasound, X-ray, usually a CT scan, urine culture, blood tests and a fitness check.
  2. Anaesthesia: our anaesthesia team usually gives general anaesthesia, or spinal anaesthesia in some patients.
  3. A thin tube is first placed into the ureter through the urinary passage to guide the operation.
  4. With X-ray or ultrasound guidance, a needle is passed from the back into the kidney and the track is widened to about 1 cm.
  5. The nephroscope is passed in, a lithotripsy device breaks the stone into small pieces, and the pieces are removed.
  6. The kidney is checked for leftover pieces, and a DJ stent and/or a small nephrostomy tube is placed if needed.

PCNL recovery time: what to expect

Most patients sit up and walk the next day and start a normal diet soon after. Some pain near the keyhole and some blood in the urine for a few days are common. If a nephrostomy tube is placed, we usually remove it in 1 to 3 days. Light work can often start in 1 to 2 weeks, but please avoid heavy lifting and hard physical work for about 4 weeks or as advised. If a DJ stent is placed, it must be removed on the date we give you, usually within a few weeks. Please drink plenty of water.

Diet and precautions after PCNL

  • Water and food: drink about 2.5 to 3 litres of fluid a day unless told otherwise, so the urine stays pale yellow. Start with light home food and move to your normal diet. Eat less salt, pickles and packaged snacks, and do not overeat meat.
  • Wound and tube care: keep the keyhole dressing clean and dry. If a tube is still in place when you go home, keep the bag below the level of the kidney and do not pull on it. Bathe as our team advises.
  • Activity: walk every day. Light work usually starts in 1 to 2 weeks. Avoid heavy lifting, cycling on rough roads and hard physical work for about 4 weeks or as advised.
  • Stent: please keep the DJ stent removal date we give you.
  • Warning signs: please come to us urgently for fever with chills, heavy bleeding or clots in the urine, severe pain, leakage from the wound or if you cannot pass urine.
