What is an abscess, and what is incision and drainage?

An abscess is a painful collection of pus under the skin or in deeper tissue, caused by a bacterial infection. It starts as a red, hot, tender swelling that becomes soft as pus collects. Abscesses are common in the armpit, on the buttocks, around the anus, in the breast of feeding mothers, and on the arms and legs. Small boils may burst and heal on their own, but a larger abscess usually does not heal with antibiotics alone.

Incision and drainage (I&D) is a small operation in which our surgeon makes a cut over the abscess, lets out all the pus, breaks the pockets inside and cleans the cavity. We then pack the cavity with gauze and dress it until it heals from the inside out.

Signs of an abscess or boil with pus

  • A red, hot, painful swelling that is getting bigger
  • The swelling becomes soft in the middle, sometimes with a yellow or white head
  • Throbbing pain that disturbs sleep
  • Fever or chills
  • Pus leaking from the swelling
  • Repeated boils, especially in people with diabetes

When does an abscess need drainage?

We advise I&D when a swelling has become soft and filled with pus, or when an ultrasound shows a pus collection. In the early stage, when there is only redness and hardness without pus, antibiotics, warm compresses and rest may be enough. Our doctor decides after examination.

Please come to us quickly if the swelling is spreading, you have fever or chills, the abscess is on the face, near the anus or in the breast, or if you have diabetes. Do not press, squeeze or cut the abscess at home, and do not apply home remedies to it, as this can spread the infection.

Dressing, diet and precautions after drainage

Pain usually eases quickly once the pus is out. Healing then depends on good care:

  • Hygiene: keep the area clean and wear loose cotton clothes. For an abscess near the anus, warm water sitz baths help, if advised.

In the early stage, when there is only redness and hardness, antibiotics and warm compresses may help. Is 'phode ka ilaj' possible without cutting?
