Uterine prolapse means the uterus (womb) slips down from its normal place into the vagina. In more advanced cases it comes out of the vaginal opening. It happens when the pelvic floor muscles and ligaments that hold the uterus in place become weak. It is common in older women, especially after menopause, after many or difficult deliveries, and in women who do heavy work or have a long-standing cough or constipation. The bladder or rectum may also sag along with it.
The operation we commonly do for this is vaginal hysterectomy with pelvic floor repair (VH + PFR). We remove the uterus through the vagina, then repair and tighten the weak front and back walls of the vagina so that they support the bladder and rectum. There is no cut on the abdomen. Many women feel shy about prolapse and live with it for years. It is a common condition and help is available, so please talk to us without hesitation.


We diagnose prolapse with a gentle examination. We also do blood tests, a urine test, ultrasound, and fitness checks such as ECG and X-ray. These checks are especially important for older women with diabetes, blood pressure or heart problems.
Not every prolapse needs surgery. Mild prolapse that causes little trouble can be managed with pelvic floor exercises and treatment of cough and constipation. A pessary, a soft ring placed in the vagina to hold the uterus up, is an option for women who cannot have or do not want surgery. We usually advise VH + PFR when the prolapse is troublesome and the woman has completed her family. For a younger woman who wants to keep her uterus, we may discuss other operations. Most women walk the next day and eat normally. Some vaginal discharge or light spotting for a few weeks is normal. Please tell us at once if you have heavy bleeding, fever or a foul smell. For about 6 weeks, avoid lifting heavy weights, squatting for long periods and straining. Drink enough water and eat fibre to prevent constipation. Avoid sexual intercourse for about 6 weeks, or as we advise. If you have a long-standing cough, please get it treated. Diet: eat normal home-cooked food with dal, green vegetables, fruit and plenty of water. Fibre and water prevent constipation, which matters because straining puts pressure on the repair. If you have diabetes, keep your sugar under control.
Activity: start walking from the next day. For about 6 weeks, avoid lifting heavy weights, squatting for long periods and straining. Avoid sexual intercourse for about 6 weeks, or as we advise. Please get any long-standing cough treated. The pelvic floor exercises we teach you at follow-up help keep the repair strong.
Hygiene: some vaginal discharge or light spotting for a few weeks is normal. Keep the area clean, and bathe as our team advises.

Like every operation, VH + PFR has possible risks. These include bleeding, infection, difficulty passing urine for some days, urine infection and, rarely, injury to the bladder, ureters or rectum. Some women may have pain or discomfort during intercourse after the repair. In some women the vaginal walls or the top of the vagina can sag again over the years, especially with heavy work, cough or constipation. Older women also have higher risks from anaesthesia and blood clots. For this reason we check fitness carefully before surgery and encourage early walking after it.
